Contact Lynda Pullen one term prior to applying for license to review your file.

You will need the following items in your file:

• Unofficial transcript from PSU that includes all of the required BTP coursework.
• Official transcripts from all other colleges and universities
• Original score reports from all professional exams
• Anti Discrimination workshop certificate
• Work sample evaluations
• Student teaching evaluation forms
• Copy of first aid card

Go to the TSPC website: www.tspc.state.or.us and link to “forms”. Print the Program Completion Form C-2, complete the top portion, and return to Lynda Pullen. Include your email address on the top of the form.

After your file is complete and you have turned in the C-2 form, go to the TSPC website and print the Fast Track Application for Educator License (C-1) form. Fill out the form and then assemble the following:
• Fast Track Application for Educator License (C-1) form
• Copy of First Aid Card if you apply after 9/30/07
• Check to TSPC for $100.00

Put these items in a single envelope and mail to:
TSPC, 465 Commercial Street NE, Salem, Oregon 97301

Once you have been recommended by PSU to TSPC for your Initial License, you will receive the following email.
From: Oregon Teacher Standards and Practices Commission
Ref: E-Mail Acknowledgement (#####)
“We have received the following document pertaining to your account:
Program Completion Report (Form C-2)”
Your correct email address must be on your Form C-2 to receive this email.

You should be licensed within 10 working days of TSPC receiving your application packet. You can check the status of your application and license by going to the “License Inquiry” link and following the instructions to do a status look up. You will eventually receive your license in the mail.
